KeithLee I thought a little modification to quiz can be used as a survey module, but conditioning it to grade does not seem clear.
Indeed, there are no options to condition a quiz to grade visibility.
I assume that by "grade" you mean final grade (not gradebook grade).
Final grades are entered manually by teachers.
Why not keeping the grade empty until the quiz has been taken?

Maybe I did not understand your request, but you can already combine assignment submission and a quiz for the same assignment.
Generally speaking, a quiz is tied to an assignment, not the final grade, so what you are asking would fall under custom development.
